IQuity Patient Payment Portal

Thank you for choosing IQuity. You have been directed to the patient payment portal because your healthcare provider has successfully ordered a diagnostic test for you. 

To begin the process, please enter the 11-digit code (Requisition ID that is formatted: "R" and 10 numbers - include all 0s) provided to you by your provider and in the email you received from IQuity, your date of birth, and your last name.

Once you have verified your contact and shipping information, you will be given the opportunity to immediately pay for the test. Please have your payment information ready. 

Accelerated payment will result in a $200 discount  to the cash price of your test (totaling up to a $400 discount, with cash payment discount). You will have 7 days to take advantage of this discount after your first login to the payment portal.

By logging into the IQuity Patient Payment Portal, I agree to receive emails, phone calls, and text messages from or on behalf of IQuity, including calls and messages using automated dialing technology. I understand that email messages from IQuity may disclose or suggest that I am eligible for or have received a laboratory test, but will not contain any test results, medical or diagnostic information or other identifying information. I acknowledge that unencrypted emails may be intercepted, received by unintended parties, or altered or used without authorization.
